The implant approach for elderly person: the technical modification and case series report

Abstract


Purpose: The purpose of this case series was to show the alternative approach for elderly persons who are in bad condition to perform implant surgery and prosthetic works. The surgery technique and prosthetic method were modified because patients had systemic disease, chronic sinus infection, bad experience about former implant surgery, etc.

Materials and Methods: The present series included 6 treated patients commonly suffering from diabetes and
hypertension. The first case has maxillary pneumatization and chronic sinusitis; the second case has bony rupture in sinus wall; the third case has extremely sensitive mandibular tissue. 2 people were unusually anxious about dental surgery, and 4 people took baby aspirin. All cases were focused on maintaining the chewing ability at any time before and after surgery, and keeping the esthetics for social activity temporarily.

Results: All implants were survived and didn¡¯t have any trouble for prostheses. All cases were completed with implant and fixed prosthetic crown and bridges. Two cases used CAD CAM titanium milling custom abutments. One patient got the implant using flapless surgery and his old denture was modified to temporary crown and bridge. Natural teeth were jointed with implant in two cases. In this case, teeth were covered with gold inner cap. The inner caps were cemented with Fuji cement. Out crowns were glued with Premier implant resin cement. All fixed prosthetic cases were fabricated with cement retained implant crowns.

Conclusion: Aging comes hand in hand with the increase in loss of teeth, which leads to the heightened possibility of nutritional deficiency. This can all result in slowing down recovery from illness. Aging also increases the possibility of applying RPD or complete denture. At this point, making of denture properly incorporating implant can be helpful for both the patient and the dentist. It is important in dentistry to help the elderly recover their chewing ability, for it directly leads to maintenance of health and the quality of life. Fixed prosthesis, especially, has greater strengths than the usual Removable prosthesis. While fixed prosthesis requires implant placement procedures, it is often the case that the elderly cannot take this surgery due to psychological or health-related reasons (systematic or local). It is in this case that modified surgical procedures can be applied, or that fixed prosthesis can be made using the modified prosthetic methods.
